The left-back had only been fit for a spot on the bench in this huge fixture at the Stadio Olimpico and came on to help his teammates at the 68th minute.
After the Taty Castellanos back-heel flick in the first half, the Aquile needed to win 2-0 in order to force extra time, securing that goal at the 93rd with Tijjani Noslin bundling over the line from a corner.
However, just three minutes into extra time, Nuno Tavares felt the all too familiar twinge of another muscular injury and burst into tears on the pitch.
Teammate Matteo Guendouzi tried his best to console the Portugal international, while the crowd also chanted his name in reassurance as he left the field to make way for Elseid Hysaj.
Sadly, muscular injuries have been the bane of his career, first at Arsenal and now with Lazio, as this was the second time that he tried to make a return as a substitute only to be forced off again.
